Five questions. Answer the ones you have an answer to and leave the rest — a page with one honest line on it is worth more than five hedged ones.

No account, and nothing is saved unless you ask for it to be. We put nothing on the finished page except your own words — no tickers, no amounts, nothing about what you own. Write it so you would not mind showing it to someone.

What is this money for? In your own words. Not a figure — what it is meant to do. How long before you draw on it? Roughly. “Twenty years” and “not for a very long time” are both answers. What do you believe about investing that shapes how you do it? The thing you would say to someone who asked why you do it your way. When something drifts away from what you intended, what do you want to happen? Including “nothing”. Doing nothing on purpose is a policy, and a common one. What would make you change your mind? The question this whole thing is built around. One line is enough.
